Semua level personnel dalam NDE harus dikualifikasi dengan prosedur ini. This procedure establishes the control and administrative requirements for the qualification and certification of NDE personnel. All levels of NDE personnel shall be qualified in accordance with this procedure. 2 Ruang Lingkup 2 Scope Qualifikasi dan sertifikasi ditujukan untuk semua Personnel NDE yang bekerja untuk PT. KEHAN SUPPLY: the corporate, private, or public entity, which employs personnel for wages, salary, fees, or other considerations. 4.8 Experience: aktifitas - aktifitas pekerjaan yang dilaksanakan pada metode NDE tertentu dibawah pengarahan supervisi yang berkualifikasi termasuk performance metode NDE dan berhubungan dengan aktivitas2, tetapi tidak termasuk waktu mengatur program training. 4.8 Experience: work activities accomplished in specific NDE method under the direction of qualified supervision including the performance of NDE method and related activities, but does not include time spent organizing the training program. 4.9 Non-destructive Testing ( NDT) : Proses yang melibatkan kegiatan inspeksi, pengujian, atau pengevaluasian dari cacat material, properti dan masalah permesinan tanpa harus merusak ataupun menghancurkan kegunaannya. 4.9 Non-destructive Testing ( NDT) : a process that involves the inspection, testing, or evaluation of materials discontinuities, properties and machine problem without further impairing or destroying the parts serviceability. 4.10 Outside Agency: Perusahaan atau personnel yang menyediakan jasa NDE Level III dan yang kualifikasinya untuk jasa ini telah direview dan disetujui oleh Director PT. KEHAN SUPPLY secara detail tentang persyaratannya untuk kualifikasi dan sertifikasi Personnel NDE. 4.13 Written Practice: a written procedure developed by the PT. Seorang trainee harus diijinkan bekerja didampingi atau dibantu yang bersertifikat tetapi tidak diperkenankan secara mandiri melakukan, interpret, evaluasi atau membuat report dari uji NDT An individual in the process of becoming qualified certified to a Level I. A Trainee shall be allowed to work with or assist a certified individual but shall not be allowed to independently conduct, interpret, evaluate or report the result of any NDT Test. 5.3 NDE Level I 5.3 NDE Level I Level I NDE harus dikualifikasi untuk bisa dengan benar melakukan kalibrasi, examination khusus, dan evaluasi khusus sesuai dengan instruksi tertulis dan merecord hasilnya. An NDE Level I personnel shall be qualified to properly perform specific calibrations, specific test, and specific evaluations according to written instructions and to record the results. Seorang personnel NDE Level II thickness gauging harus familiar dengan intrumentasi, peralatan dan keterbatasan dari metodenya dan harus qualified untuk mengeset dan mengkalibrasi peralatan dan menginterpret, mengevaluasi hasil examination dengan pedoman Code yang berlaku, Standard dan Spesifikasi. e. An NDE Level II personnel for thickness gauging shall be thoroughly familiar with the instruments, equipment and limitations of the method and shall be qualified to set up and calibrate equipment and to interpret, evaluate test results with respect to applicable codes, standards and specifications. 5.5 NDE Level III 5.5 NDE Level III a. Level III NDE harus bisa membuat, mengqualifikasi, dan mengesahkan prosedur, menetapkan dan mengesahkan teknik pelaksanaan, menginterpretasikan Code, Standard, specifikasi dan dan prosedur; dan merencanakan metode NDE yang penting, teknik dan prosedur yang dipakai. a. An NDE Level III personnel shall be capable of developing, qualifying, and approving procedures, establishing and approving techniques, interpreting codes, standards, specifications and procedures; and designating the particular NDE methods, technique and procedure to be used. b. An NDE Level III personnel shall be authorized to administer the training and certification program in accordance with this procedure. 6 Pendidikan, Training dan Pengalaman 6 Education, Training and Experience Requirements 6.1 Sertifikasi personnel Level I dan II NDE 6.1 NDE Level I & II certified personnel shall KS PT. Personnel yang bersertifikasi NDE Level I and II harus memiliki pendidikan, pengalaman dan pelatihan sesuai dengan Tabel 1. a. NDE Level I and II certified personnel shall satisfy the education, experience and training requirements of Table 1. b. Untuk kualifikasi langsung ke Level II, jam pelatihan harus sedikitnya adalah kombinasi jam yang dipersyaratkan untuk Level I dan II pada metode yang berlaku, terkecuali bila calon tersebut telah terkualifikasi sebelumnya, atau memiliki persamaan posisi sesuai dengan Level II, dalam kasus ini persyaratan untuk pelatihan dinyatakan telah terpenuhi. b For Level II direct qualification with no time at level I, the training hours shall consist of at least the combined required hours for Level I and II in the applicable method, except when the candidate has either been previously qualified to, or has held a position equivalent to that of a Level II, in which case, the requirement for training may be considered met. 6.2 Sertifikasi personnel Level III NDE harus mengikuti item a dan salah satu kriteria pendidikan ataupun pengalaman ( b, c or d) : 6.2 NDE Level III certified personnel shall satisfy below item a and one of the following education and experience criteria ( b, c or d ) : a. Memiliki jam training minimum jumlah dari level I dan level II sesuai metode, seperti yang tercantum dalam table 1 a. Have the training hours minimum the sum of Level I and Level II in applicable method, as stated in the Table 1. b. Lulus dalam 4 tahun di perguruan tinggi atau Universitas dengan tingkat sarjana atau science , plus 1 tahun pengalaman melebihi syarat2 Level II NDE dalam penugasan yang sebanding dengan Level II NDE pada metode yang digunakan, atau: b. Have graduated of four ( 4) year college or university with degree in engineering or science , plus one ( 1) additional year of experience beyond the level II requirements in NDE in an assignment comparable to that of an NDE Level II in the applicable method( s) , or : c. Telah lulus, tidak kurang dari dua tahun belajar di bidang engineering atau science pada universitas yang terakreditasi, perguruan tinggi atau sekolah teknik ditambah 2 tahun pengalaman melebihi syarat2 Level II NDE dalam penugasan yang sebanding dengan Level II NDE pada metode yang digunakan, atau: c. Have Completed with passing grades at least two years of engineering or science study at an accredited university, college, or technical school, plus two ( 2) additional years of experience beyond the level II requirements in NDE in an assignment comparable to that of an NDE Level II in the applicable method( s) , or : d. Telah mempunyai pengalaman empat ( 4) tahun melebihi syarat2 Level II NDE dalam penugasan yang sebanding dengan Level II NDE pada metode yang digunakan. d. Have four ( 4) years of experience beyond the level II requirements in NDE in an assignment comparable to that of an NDE Level II in the applicable method( s) . 6.3 Untuk meyakinkan bahwa personal tersebut telah memahami mengenai materi pelatihan, dia harus memenuhi syarat2 uijian pada paragraf 8, yang cocok. 6.3 To assure that an personnel has understood the training material presented, he shall satisfy the examination requirements of paragraph 8, as KS PT. KEHAN SUPPLY Serving industry, mining and oilfield Procedure No : 01 – QCP - KS Revision : 00 WRITTEN PRACTICE FOR QUALIFICATION AND CERTIFICATION OF NON DESTRUCTIVE EXAMINATION PERSONNEL Date : January 26, 2012 Page No. : 8 of 26 applicable. 6.4 Record digunakan untuk menunjang pendidikan, pelatihan dan pengalaman harus teridentifikasikan dan disimpan sesuai paragraf 9.3. 6.4 Records used to substantiate education, training and experience shall be identified and maintained in accordance with paragraph 9.3. 7 Program Pelatihan 7 Training Program 7.1 Personel yang terpilih ( trainee) akan diberikan pelatihan praktek, program pengembangan dan perlakuan pada pengujian tak merusak dan untuk memenuhi pengalaman yang cukup untuk memenuhi waktu yang disyaratkan pada spesifik teknik, trainee kemudian dikembalikan ke NDE Level III. Pada saat ini, trainee akan diberikan kelas pelatihan untuk metode dimana dia akan dikualifikasikan ( Kelas pelatihan dapat bersamaan dengan trainee mendapatkan pengalamannya) . 7.1 Personnel selected ( trainee) are given practical training, development and aptitude in nondestructive examination, and gain experience for the appropriate time for meeting the general time guide in the specific technique; the trainees will be referred back to the NDE Level III. At this time, trainees will be given the classroom training ( classroom training may also be given simultaneously as the trainee gains practical experience) for the method in which he will qualify. 7.2 Procedur diatas diikuti dalam pemilihan, training dan kualifikasi personel yang tidak memiliki pengalaman pengujian tak merusak sebelumnya. Setelah menunjukkan bukti training sebelumnya dan/ atau kualifikasi, NDE level III dapat melakukan penilaian terhaddap program training untuk latar belakang dan pengalaman personel. Tipe pengujian specific yang didefinisikan untuk setiap NDE teknik akan diikuti dalam semua kasus. 7.2 The above procedure will be followed in selecting, training and qualifying personnel with no previous experience in nondestructive examination. Upon presentation of proof of prior training and / or qualification, the NDE Level III may exercise judgement in tailoring the training program to an personnel’ s background and experience. The specific types of qualification testing as defined for each NDE technique will be followed in all cases. 7.3 Program pelatihan harus sesuai dengan SNT – TC -1A dan Program Pelatihan NDE ANSI/ ASNT CP-105 untuk setiap metode sebagai syarat minimumnya. KEHAN SUPPLY dan hanya dikeluarkan nilai dengan kriteria lulus atau tidak lulus, pada sertifikatnya, maka PT. KEHAN SUPPLY bisa menerima dengan dinyatakan telah lulus dengan nilai 80 persen untuk ujian tersebut. 8.1.5 When an examination is administrated and graded for the by an outside agency and issues grades of pass or fail only, on a certified report, then PT. KEHAN SUPPLY may accept the pass grade as 80 percent for that particular examination. 8.2 Pengujian penglihatan: 8.2 Vision Acuity Examination : 8.2.1 Pengujian diperlukan untuk meyakinkan penglihatan dekat baik normal ataupun dengan kaca mata ninimum satu mata bisa membaca tulisan huruf J-2 dengan jarak minimum 30 cm pada Tes Chart Standar Jaeger. 8.2.1 An examination is required to assure natural or corrected near vision in at least one eye by reading J-2 letters at a distance not less than 12 inches on a standard Jaeger test chart 8.2.2 Sebagai tambahan NDE Level III harus yakin bahwa kandidat tersebut memiliki kemampuan untuk membedakan kontras diantara warna2, menggunakan Ishihara’ s Test Plates. 8.2.2 In addition the NDE Level III shall satisfy himself of the candidate’ s ability to distinguish and differentiating contrasts among colours, using Ishihara’ s Test Plates ( minimum 14 plates ) . 8.2.3 Untuk personnel RT dan MT harus bisa membedakan bayangan kelabu, menggunakan copy atau computer dari Shade Grey Test Plates dengan jarak 30 ~ 8.2.3 For NDE personnel on RT and MT, they shall be able to distinguish and differentiating shades of grey, using a hard copy or on computer screen of Shade KS PT. The general examination shall cover the basic test principles relative to the applicable method. Pertanyaan ujian harus dibuat menggunakan pertanyaan pada SNT – TC- - 1A dan NDE Training Program ANSI / ASNT CP – 105 sebagai panduan untuk metode RT, UT, MT dan PT. The examination questions shall be developed using the questions in the appropriate supplements to SNT – TC- - 1A and NDE Training Program ANSI / ASNT CP – 105 as a guide for RT, UT, MT and PT. The practical examination shall comprise demonstrated proficiency in performing the applicable nondestructive examinations and evaluations of the results obtained. b. The specific examination in the applicable method may be deleted for a candidate who has a valid endorsement on an ASNT Level III or ACCP Professional Level III certificate in the method and if documented evidence of experience exists, including preparation of procedures to Codes, standards, or specifications and evaluation of the test results. 8.4 Ujian - ulang 8.4 Re-examination Batasan dibawah ini harus dipakai syarat ujian ulang dari personnel dalam kualifikasi uji nondestructive: The following limitations shall be applied on re-examination of personnel for qualification in non-destructive examination : Personnel gagal memenuhi grade yang dibutuhkan harus menunggu paling sedikit 30 hari atau menerima tambahan training yang cocok ditentukan oleh Level III penguji sebelum dilakukan pengujian kembali. Personnel failing to attain the required grade must wait at least 30 days or receive suitable additional training as discretion by the Level III examiner prior to being re-examined. 9 Sertifikasi 9 Certification 9.1 Setelah berhasil melewati ujian kualifikasi , personnel tersebut harus disertifikasi sesuai dengan metode dan level untuk kualifikasinya. Sertifikat qualifikasi untuk RT, UT, PT, MT harus di tanda tangani oleh Director dan diverifikasi oleh NDE Level III metode yang terkait. Sertifikasi ( Exhibit B atau form serupa) harus disimpan kedalam file nondestructive 9.1 Upon successful completion of qualification examination, the personnel shall be certified in the method and to the level for which he is qualified. The qualification certificate for RT, UT, PT, MT shall be verified by the NDE Level III in applicable method and shall be certified by the Director. The certification record ( Exhibit B or similar form) shall be maintained in the nondestructive examination personnel records file. 9.2 Personnel NDE Level I or II for PT, MT, UT dan RT yang bergabung keperusahaan yang telah dikualifikasi oleh Institusi diluar dan dapat menunjukkan sertifikat authentic telah memenuhi persyaratan qualifikasi dari PT. Bila NDE level III dari subkontrak, Director harus menyimpan record sertifikasi. Record personnel yang qualified akan termasuk tersebut dibawah ini: 9.3 The NDE Level III shall maintain the certification records. If NDE Level III Services are subcontracted, the Director shall maintain records of certification. Personnel records of certified personnel will include the followings : 1. Nama dari personnel tersebut. 2. Level dari sertifikasi dan metode examinationnya 3. Latar belakang pendidikan dan pengalaman kerjanya. 4. Pernyataan memuaskan penyelesaian complete training. 5. Bukti lain untuk kecukupan qualifikasi bila kualifikasi digunakan sebagai pengganti ujian specific sesuai par. 8.3.5 ( b) . 8. Bobot persen rata – rata atau bukti bobot yang sesuai. 9. Tanda tangan Level III yang memverifikasi qualifikasi dari calon untuk sertifikasi. Untuk sertifikasi level III, harus dilakukan oleh Director. 10. Tanggal sertifikasi dan/ atau resertifikasi dan penunjukkan untuk NDE. 11. Tanggal berlakunya sertifikat. 12. Tanda tangan employer berwenang yang mensertifikasi sertifikasi ( Director) 1. Name of certified personnel. 2. Level of certification and test method. 3. Education background and experience of certified personnel. 4. Statement indicating satisfactory completion of training. 5. Result of current eye examination 6. Current examination copy( ies) or evidence of successful completion of examinations. 7. Other suitable evidences of satisfactory qualifications when such qualifications are used in lieu of the specific examination prescribed in par. 8.3.5 ( b ) . 8. Composite grade( s) or suitable evidence of grade. 9. Signature of Level III that verified qualifications of candidate for certification. For level III certification, it shall be by Director 10. Date of certification and / or recertification and date of assignments to NDT. 11. Certification expiration date. 12. Signature of PT. Evaluasi dan dokumentasi mengikuti format dan petunjuk sesuai paragraph 8.3.4. Technical performance evaluation shall be conducted annually using practical method questions ( with at least 10 check points) related to the applicable NDE method by the Level III. The evaluation and documentation shall follow the format and guidelines described in paragraph 8.3.4. 11 Interupsi Pelaksanaan 11 Interrupted Service 11.2 Demonstrasi yang tidak berlanjut terhadap service, pengetahuan atau performance keterlambatan dalam sertifikat perpanjangan waktu kualifikasi ( table 1) . Dalam keadaan yang serupa atau sama diatas terjadi, NDE level III akan mereview secara detail keadaan secara specific dan menentukan kursus yang diperlukan atau mencabut sertifikat, ujian ulang atau melakukan evaluasi lanjuta terhadap performance praktek personel. 11.1 Lack of continued demonstration of satisfactory service, knowledge or performance and delays in certification due to extended experience qualifying time ( Table I) . In the event that circumstances similar to or the same as above arise, the NDE level III will review in detail the specific circumstances and determine the necessary course or action to revoke certification, required reexamination or conduct extended evaluation of the personnel’ s practical performance. 11.2 Tidak dalam kondisi yang sesuai 8.4 ( reexamination) , periode dimana terdapat interupsi pelaksanaan yang lebih dari 6 bulan harus diadakan evaluasi ulang terhadap kompetensi seseorang oleh NDE Level III. 11.2 Not with standing the provisions of 8.4 ( Re-examination) , any period of interrupted service more than 6 months shall require re – evaluation of the personnel’ s competence by the NDE Level III. 12 Re-sertifikasi 12 Re-certification Semua level dari personnel NDE harus disertifikasi ulang sesuai dengan salah satu dari kriteria-kriteria berikut ini: All levels of NDE personnel shall be recertified in accordance with one of the following criteria : a. Adanya bukti kontinyuitas evaluasi tehnik kinerja yang memuaskan untuk level I dan II sesuai paragraph 10. Hasil yang tidak memuaskan dari technical performance, personel NDE harus direview untuk evaluasi lebih lanjut oleh level III dan Director. Evidence of continuing satisfactory technical performance of NDE personnel level I and II as described in paragraph 10. Unsatisfactory results of technical performance , the NDE personnel shall be reviewed for further evaluation by the Level III and the Director. KEHAN SUPPLY. 13.1 Personnel certified under this procedure shall have their certification revoked upon termination of employment with PT. KEHAN SUPPLY. Personnel yang sertifikasinya berakhir, boleh disertifikasi ulang sebagai level NDE seperti yang lalu oleh PT. KEHAN SUPPLY berdasarkan pada ujian, sesuai paragraf 8, asalkan semua persyaratan seperti berikut bisa terpenuhi: 13.2 Personnel whose certification has been terminated may be re-certified to the former NDE level by PT. KEHAN SUPPLY based on examination as described in par. 8, provided all of the following conditions are met to the PT. KEHAN SUPPLY satisfaction : • Personnel telah menyetujui sebelum adanya sertifikasi • Terus bekerja dalam kapasitasnya sesuai sertifikatnya dalam waktu enam ( 6) bulan setelah masa berhentinya. • Personnel disertifikasi ulang dalam waktu enam ( 6) bulan setelah masa berhentinya • Sebelum pengujian untuk sertifikasi, personnel yang tidak memenuhi persyaratan tersebut diatas harus mendapatkan tambahan pelatihan yang cukup dari NDE Level III. • The personnel has proof of prior certification • The personnel was working in the capacity to which certified within six ( 6) months of termination. • The personnel is being re-certified within six ( 6 ) months of termination • Prior to being examined for certification, personnel do not meet the above requirements shall receive additional training at the discretion of the NDE level III. 14 Pembaruan 14 Reinstatement 14.1 Personnel yang sertifikasinya telah habis, bisa diperbarui sesuai NDE level awalnya, tanpa adanya ujian baru, asalkan semua persyaratan berikut terpenuhi: 14.1 Personnel whose certification has been terminated may be reinstated to the former NDE level, without new examination, provided all the following conditions are met: 14.1.1 Pengusaha tetap menyimpan record sertifikat personnel yang dibutuhkan di paragraph 9.3 14.1.1 PT. All individual shall demonstrate his proficiency in producing radiographs in at least two different techniques. At least one radiograph to be produced for each technique inclusive of processing. 2. This examination will be graded for the following items. i. Exposure time calculation. ii. Quality of radiographs. iii. Compliance with testing procedure / technique sheet provided. iv. Knowledge on equipment and materials used where the individual is requested to fill in a format on equipment / material used and reporting. 3. All radiographs will be interpreted and sensitivity calculated by the individual being qualified. The individual’ s comment on the qualities of weldment and radiographs shall be considered. No major indications shall be missed. ( For Level II only ) 4. At least ten check points prepared and approved by the Co. NDT Level III shall be used as a basis in grading of practical examination. 5. With adequate experience and written proof of certification by an audited outside agency or national certifying body, i.e. Indonesia Atomic Energy commission ( BATAN ) , the Co. NDT Level III may waive examinations for radiographic testing personnel. This clause is applicable only for radiographic inspection personnel operating radiation sources with a detailed operating procedure under the local statutory requirements inside Indonesia EXHIBIT D ( continued) GRADING PRACTICAL EXAMINATION ULTRASONIC TESTING 1. Categories of Qualification: Ultrasonic testing certification shall be categorized according to weld joint configuration. If none is specified in the certificate, it shall mean the individual is qualified for all configurations or materials. CATEGORY NO. The individual being examined shall submit a written report in a standard format, of ultrasonic test with a sketch containing the nature, size ( length and width) , location along the weld and position within the weld cross section, of all reportable indications. 3. The individuals performance rating shall be calculated in accordance with the following formula : P = ( Lc / La) x 100 R = ( Lc / Li) x ( 1 – ( Lf / Li) x 100 Where : P = R = La = Lc = Li = Lf = Percentage of reflector correctly detected and sized. Passing grade 70% overall rating including penalty for false alarms. Passing grade 50% the length of actual reflector contained in the specimen The sum of indicated length of reflectors that have been correctly sized and located. ( Credit is given for the lesser of the reported length or actual length of the reflector) The total length of reflectors reported by the candidate, right or wrong. The total length of reported indications where no reflectors exist 4. Additionally, the individual shall be subjected to oral test where knowledge on equipment check and calibration, flaw sizing technique, codes/ standard, practical problem likely to be encountered, limitations of the test system, will be assessed. 5. At least ten check points prepared and approved by the NDT Level III shall be used as basis in grading of practical examination. 6. Practical examination may be carried out on production welds counter checked by the Co. NDT Level III or his designee. KS PT. At least one ( 1 ) specimen shall be used for examination. 2. All indications, relevant or non-relevant shall be reported, on a standards report format including comments on indications found. 3. Oral test shall be conducted to assess the individual’ s knowledge on the limitation of the test system, techniques and procedures used, code / standard, and equipment sensitivity calibration. 4. In cases where test specimens containing known discontinuities are not available, specimens without discontinuity may be used where the test technique will be assessed. 5. At least ten check point prepared and approved by the Co. NDT Level III shall be used as a basis in grading of practical examination. 6. Practical examination may be carried out on production welds witnessed by the Co. NDT Level III or his designed examiner.
